USMAHOF Nomination Form

You may select only ONE category when nominating a martial arts Black Belt instructor, competitor, school owner, school, or organization. We also have special categories for Law Enforcement, First Responders, and Military Personnel. Self-nomination is prohibited. Please include an explanation as to WHY this individual (or organization) should be considered for nomination. ALL submissions will be reviewed, vetted, and approved by our Selection Committee. Nomination is no guarantee any individual or organization will be recognized and the USMAHOF reserves the right to deny induction to anyone.
